This form is to be read, signed and returned to the Midnapore Lake Office by any Midnapore Lake Member over the age of 18 who wishes to use any of the boats provided by the Midnapore Lake Residents Association.

This form is to be printed off, completed and submitted to the Midnapore Lake office whenever a group with more than 15 guests is being brought into the Midnapore Lake Park. This form is also used to reserve either of our two south cook houses (please see the booking calender for availability prior to attempting to book).
